Job Summary

A company is looking for a Case Manager, Registered Nurse.

Key Responsibilities

• Engage with members telephonically to coordinate their care and develop individualized care plans
• Provide education and support for disease management and connect members with resources
• Document care management activities accurately and participate in interdisciplinary team meetings

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in Kentucky or compact licensure
• Proficient in Microsoft Office Suite
• Access to a private workspace for remote work
• 3+ years of nursing experience
• 2+ years of case management or home healthcare coordination experience
