Kentucky Licensed Clinical Pharmacist

Job Summary

To improve disease state outcomes and medication safety, the full-time Kentucky Licensed Clinical Pharmacist will manage high-risk patient populations remotely, providing clinical recommendations and collaborating with healthcare providers while working Monday through Friday, 8:00 AM to 5:00 PM.

Key responsibilities

• Support quality initiatives to enhance HEDIS metrics related to medication adherence and routine health visits
• Provide clinical recommendations on medication management, including compliance, drug interactions, and therapeutic substitutions
• Collaborate with prescribers to review patient drug profiles and ensure safe, effective medication therapy

Required qualifications

• Bachelor's degree or advanced degree (Pharm.D., M.S.) in pharmacy
• 2+ years of experience in mail order, retail, hospital, managed care pharmacy, or pharmacy residency program
• Current Kentucky pharmacy license
