Job Summary

To support a pioneering healthcare team, the full-time Kentucky Licensed Family Nurse Practitioner will conduct virtual wellness visits, perform health assessments, and deliver a wide range of healthcare services to patients in their homes.

Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home team members to evaluate vital signs and coordinate patient care
• Provide comprehensive care management, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Kentucky
• Proven experience performing wellness visits
• Technologically savvy with experience using HIPAA secure messaging apps
• Ability to pass a national background check and valid clinical license search
