Job Summary
To support members under state-appointed adult guardianship, the full-time Kentucky Licensed Guardianship Liaison will facilitate care management enrollment, coordinate medically necessary services, and collaborate with interdisciplinary care teams while working remotely.
Key responsibilities
- Conduct outreach to state-appointed guardians to align state-approved service plans with care management requirements
- Serve as a liaison and advocate among members, state guardians, providers, and community organizations to ensure service coordination
- Complete comprehensive assessments and develop individualized care plans that address medical, behavioral health, and social needs
Required qualifications
- Master's degree in behavioral health or social work, or a degree from an accredited school of nursing
- 2 - 4 years of related experience, with guardianship experience preferred
- Licensure as a Licensed Master's Behavioral Health Professional (e.g., CSW, LCSW, LMSW, LPCC, LPC) or RN based on state contract requirements
- Behavioral health experience is required
