Job Summary

To support adult oncology patients, the full-time Kentucky Licensed Oncology Nurse Navigator will provide care coordination, medication therapy management, and patient education while working remotely within a commutable distance to Louisville, KY.

Key responsibilities

• Communicate with patients to explain their disease, treatment plans, and potential side effects
• Provide medication therapy management and follow up with patients and healthcare providers to monitor treatment progress
• Document all communications with patients and healthcare providers, adhering to confidentiality laws and reporting any adverse drug reactions

Required qualifications

• Associate of Science in Nursing (required); Bachelor of Science in Nursing (desired)
• Minimum of 1+ years in an oncology nursing setting (required); 1+ years in a specialty pharmacy setting (desired)
• Current license as a registered nurse (required)
• Compassionate and energetic attitude with strong multi-tasking abilities (required)
• Clinical and/or case management experience (desired)
