Kentucky Licensed RN Care Manager

Job Summary

To support a growing healthcare team, the full-time Kentucky Licensed RN Care Manager will manage the development and oversight of patient care plans, collaborate with field staff, and participate in interdisciplinary meetings while working remotely.

Key responsibilities

• Review and revise Plans of Care to ensure accuracy and compliance with regulatory standards
• Manage recertification and discharge processes while adhering to utilization management guidelines
• Collaborate with clinicians and participate in interdisciplinary team meetings to enhance patient care delivery

Required qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB and knowledge of Medicare and Medicaid guidelines for patient care compliance
