Job Summary

A company is looking for a 100% Remote RN Care Navigator in Kentucky.

Key Responsibilities

• Manage case management activities specific to kidney health, including member data entry and health screenings
• Educate members on kidney health and related co-morbid conditions, optimizing renal replacement therapy
• Coordinate care as a liaison between patients, their support network, and healthcare providers

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
