Kentucky Licensed RN Case Manager

Job Summary

To support the Integrated Kidney Care National Flex Team, the full-time Kentucky Licensed RN Case Manager will autonomously coordinate quality care for ESRD and CKD patients, collaborating with healthcare teams while working remotely.

Key responsibilities

• Manage and facilitate care coordination between patients, families, and healthcare providers
• Implement integrated care strategies to improve patient outcomes and reduce healthcare delivery fragmentation
• Utilize clinical data to monitor and achieve clinical, financial, and patient satisfaction goals

Required qualifications

• A.D.N. degree from an accredited nursing program required; B.S.N. preferred
• Minimum of 1 year of experience as a DaVita IKC RN Case Manager required
• Compact Nursing License highly preferred
• Minimum of five years of clinical nursing experience required
• Current CPR certification required
