Kidney Care Options Educator

Job Summary

A company is looking for a Kidney Care Options Educator, Care Coordinator.

Key Responsibilities

• Promote and deliver the Kidney Care Options Program to nephrology offices and community partners
• Plan and conduct education sessions for CKD Stage 4 and 5 patients and coordinate care needs
• Monitor patient status and maintain communication with healthcare teams while reporting metrics to leadership

Required Qualifications

• Bachelor's degree or equivalent work experience
• Five years of experience in a healthcare organization with patients and/or physicians
• Knowledge of nephrology, End-Stage Renal Disease (ESRD), or dialysis treatment
• Preferred experience in patient education or sales for at least one year
• Bi-lingual preferred
