LCSW NICU Case Manager

Job Summary

A company is looking for a NICU Case Manager, LCSW (Remote).

Key Responsibilities:

• Empower caregivers and families to support optimal wellness and advocate for necessary services and community resources
• Assess, plan, and implement individualized care strategies directed toward appropriate levels of care
• Build relationships with caregivers and healthcare providers to develop member-focused care plans addressing physical and psycho-social barriers

Required Qualifications:

• Minimum of 2 years of experience in Pediatrics/NICU managed care or clinical environment
• Valid unrestricted Social Worker license in the state of residence, with willingness to obtain licensure in other states as needed
• Five or more years of managed care experience in a case management role for 100% telecommuting positions
• Experience with a Medicaid population is preferred
• CCM certification preferred upon hire, with the requirement to obtain it within specified timeframe
