Key Responsibilities

Completes assessments to identify member needs and barriers
Builds relationships with members and healthcare providers to create individualized care plans
Coordinates referrals for social services and monitors service delivery

Required Qualifications

Minimum of 2 years of experience in managed care or a clinical environment with a maternity population
Valid unrestricted Social Worker license (LCSW) required
Master's degree in Social Work (MSW) required
CCM certification preferred upon hire; required within specified timeframe
Experience with data entry into a database or EHR required
