Lead APP, Telehealth Assessment

Job Summary

A company is looking for a Lead APP, Virtual Health Assessment.

Key Responsibilities

• Perform preventative care screenings, medication reconciliation, and provide health coaching and treatment virtually
• Outreach to chronically ill members and engage those less likely to see their PCP
• Supervise a team of APPs and support the development of clinical guidelines and care models

Required Qualifications

• NP or PA from an accredited program
• Board Certification (NCCPA or AANP or ANCC)
• 5+ years of outpatient primary care experience
• 1+ year of telehealth experience
• 1+ years of experience supervising clinical teams
