Job Summary

A company is looking for a Lead Health Coach.

Key Responsibilities

• Provide one-on-one coaching to members managing chronic health conditions through various communication channels
• Conduct initial health assessments and document member progress in accordance with policies
• Offer training and mentorship to Health Coaches and collaborate on strategic initiatives and program development

Required Qualifications, Training, and Education

• Associate degree, preferably in a healthcare-related field
• Certification as a National Board-Certified Health and Wellness Coach (NBC-HWC)
• Minimum of four years of experience in health coaching or related fields
• At least two years of experience in disease management or health coaching within a health plan or similar organization
• Thorough knowledge of health behavior change theories and their practical application
