Key Responsibilities

Evaluate new members and formulate individualized care plans
Facilitate individual coaching through video/phone calls and app-based communication
Manage a caseload of members, monitor health outcomes, and coordinate care

Required Qualifications

Certified as an alcohol and drug counselor or equivalent
3-5 years of experience in alcohol use disorder or addiction treatment
BA/BS and/or MA preferred
Experience with EMR systems, Zoom conferencing, and app-based applications
Knowledgeable of evidence-based substance use disorder treatment programs
