Licensed Behavioral Health Care Manager

Job Summary

A company is looking for a Behavioral Health Care Navigator.

Key Responsibilities

• Support individuals with mental health conditions and substance use disorders through virtual engagement
• Navigate individuals to appropriate behavioral health and primary care services, including scheduling and follow-up
• Collaborate with the clinical team on care issues and participate in case reviews

Required Qualifications

• Hold a Registered Nurse license or Clinical Social Work license
• 3+ years of experience working for a health plan or at-risk provider
• 2+ years of experience in care coordination or case management
• 2+ years of contact center experience with patient outreach and engagement
• Demonstrated experience working with individuals facing complex chronic needs or mental health conditions
