Key Responsibilities

• Conduct comprehensive behavioral and physical health assessments
• Develop and coordinate individualized care plans for members
• Provide crisis intervention and follow-up support for members in need

Required Qualifications

• Master's degree in Social Work or Counseling
• Active, unrestricted independent clinical license in the state
• 3+ years of post-master's direct clinical experience
• 3+ years of experience with mental health and substance use disorders
• Comfort with telephonic care and real-time documentation
