Licensed Behavioral Health Care Manager

Job Summary

To support a growing team focused on improving access to behavioral health care, the contract-to-hire Licensed Behavioral Health Care Manager will engage with individuals experiencing mental health and substance use disorders, navigate them to appropriate services, and collaborate with a clinical team in a remote setting.

Key responsibilities:

• Engage and assess individuals with mental health conditions using an omnichannel virtual approach
• Identify care goals and navigate individuals to necessary behavioral health and primary care services
• Collaborate with the clinical team on care issues and ensure key engagement metrics are met

Required qualifications:

• Active Registered Nurse (RN) license or Clinical Social Work license
• 3+ years of experience in care coordination or case management within a health plan or at-risk provider
• Experience in a contact center or similar environment focused on patient outreach and engagement
• Demonstrated experience supporting individuals with complex needs, including mental health and substance use disorders
• Strong technical proficiency with CRM systems and omnichannel communication tools
