Key Responsibilities

Conducts comprehensive clinical assessments and prioritizes member needs
Develops and monitors care plans in collaboration with members and their families
Facilitates referrals to community resources and ensures documentation meets regulatory standards

Required Qualifications

Registered Nurse with psychiatric experience or a licensed Behavioral Health Professional
3 years of relevant direct clinical care experience
CCM Certification or ability to obtain within 3 years
High school diploma (or equivalency) and legal authorization to work in the U.S
Flexibility to work varied hours
