Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager Behavioral Health - SUD Focus.

Key Responsibilities

• Conducts comprehensive clinical assessments and develops plans of care to support member needs
• Collaborates with members and their families to ensure effective care coordination and resource utilization
• Monitors and evaluates the effectiveness of care plans over time and documents member data as per regulatory standards

Required Qualifications and Education

• Registered Nurse with psychiatric experience or a licensed Behavioral Health Professional
• 3 years of relevant direct clinical care experience
• CCM Certification or ability to obtain within 3 years of employment
• High school diploma (or equivalency) and legal authorization to work in the U.S
• Flexibility to work varied hours
