Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Transitions of Care Behavioral Health Social Work Case Manager.

Key Responsibilities

• Perform comprehensive psychosocial assessments via phone/video with members and caregivers
• Conduct clinical social work interventions to enhance program effectiveness and assess social determinants of health
• Collaborate with interdisciplinary teams to ensure cohesive, patient-centered care for high-needs patients

Required Qualifications

• Master's degree in Social Work with an active clinical license (LCSW, LMSW, or comparable)
• Minimum of 5 years of experience in behavioral health or care management
• Competent in performing comprehensive psychosocial assessments over video and phone
• Able to independently manage a fully remote work schedule, M-F, 8:30am-5:00pm local time
