Job Summary

Key Responsibilities

Conducts comprehensive clinical assessments and collaborates with members to develop care plans
Identifies and facilitates referrals to relevant resources and monitors care plans over time
Engages with providers to ensure alignment of treatment plans with benefits and care coordination

Required Qualifications

Registered Nurse with psychiatric experience or a licensed Behavioral Health Professional
3 years of relevant direct clinical care experience
CCM Certification or ability to obtain within 3 years
High school diploma (or equivalency) and legal authorization to work in the U.S
Flexibility to work varied hours
