Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Social Work Case Manager.

Key Responsibilities

• Coordinate care and track progress for patients with mental health and substance abuse disorders
• Provide brief behavioral interventions and patient education on treatment options
• Participate in caseload consultations and support psychotropic medication management

Required Qualifications

• Licensed Clinical Social Worker, Licensed Professional Counselor, or related field
• Completion of degree and post-degree supervision with at least 2 years of experience
• Experience delivering evidence-based psychosocial treatments and behavioral interventions
• Ability to engage patients in short-term therapeutic relationships
• Licensed in one or more states with a willingness to obtain multiple state licenses
