Job Summary

A company is looking for a Clinical Case Manager Behavioral Health DSNP.

Key Responsibilities

• Conduct comprehensive clinical assessments integrating medical, behavioral health, and psychosocial needs
• Develop, implement, and monitor individualized care plans in collaboration with members and providers
• Coordinate care with medical and behavioral health professionals to ensure high quality, cost-effective outcomes

Required Qualifications

• Master's degree in social work or counseling
• Active, unrestricted independent clinical license in the state
• 3+ years of post-master's direct clinical experience
• 3+ years of strong knowledge of mental health and substance use disorders
• 3+ years of case management experience
