Job Summary
Focusing on Transitions of Care support, the full-time Licensed Behavioral Health Case Manager will provide short-term interventions to members transitioning to community-based behavioral health services, working remotely while preventing avoidable hospitalizations.
Key responsibilities
- Perform comprehensive psychosocial assessments via phone and video with members and caregivers
- Conduct clinical social work interventions to enhance program effectiveness and address social determinants of health
- Collaborate with interdisciplinary teams to ensure cohesive, patient-centered care for high-needs patients
Required qualifications
- Master's degree in Social Work with an active clinical license (LCSW, LMSW, or comparable)
- Minimum of 5 years of experience in behavioral health or care management
- Competence in performing comprehensive psychosocial assessments remotely
- Able to manage a fully remote work schedule, M-F, 8:30am-5:00pm PST or MST
- Willingness to obtain additional state licenses as needed
