Licensed Behavioral Health Navigator

Job Summary

A company is looking for a Behavioral Health Care Navigator.

Key Responsibilities

• Support individuals with mental health conditions and substance use disorders through virtual outreach and engagement
• Screen for unmet behavioral health, social service, and physical health needs while identifying barriers to care
• Facilitate navigation to appropriate care, including behavioral health and primary care services

Required Qualifications

• 2+ years of experience in a patient-facing healthcare setting, with a preference for Community Health Worker or licensed social work certification
• 1-2 years of experience in patient outreach and engagement
• 2+ years of experience in a metric-driven environment
• Access to a quiet, HIPAA compliant workspace
• Experience with individuals facing complex chronic needs, including mental health and substance use disorders
