Job Summary

A company is looking for a Behavioral Health Care Navigator.

Key Responsibilities

• Support individuals with mental health conditions and substance use disorders through virtual outreach and engagement
• Perform assessments to identify clinical needs and barriers to care, providing navigation to appropriate services
• Conduct post-care follow-ups to ensure continued support and care integration

Required Qualifications

• 2+ years of experience in a patient-facing healthcare role, with preferred certification as a Community Health Worker or licensed social worker
• 1-2 years of experience in patient outreach and engagement
• 2+ years of experience in a metric-driven environment
• Demonstrated experience working with individuals facing complex chronic needs or mental health conditions
• Ability to thrive in a fast-paced, dynamic startup environment
