Job Summary

A company is looking for a Behavioral Health Care Navigator.

Key Responsibilities

• Support individuals with mental health conditions and substance use disorders through virtual outreach and engagement
• Screen for unmet behavioral health, social service, and physical health needs, and provide navigation to appropriate care
• Utilize motivational interviewing techniques to understand and address individuals' care goals and barriers to care

Required Qualifications

• 2+ years of experience in a patient-facing healthcare setting, with a preference for Community Health Worker or licensed social work certification
• 1-2 years of patient outreach and engagement experience
• 2+ years of experience in a metric-driven environment
• Access to a quiet, HIPAA-compliant space for clinical duties
• Demonstrated experience working with individuals facing complex chronic needs or mental health conditions
