Licensed Behavioral Health Navigator

Job Summary

Seeking a full-time Contractor Licensed Behavioral Health Navigator, this remote position will manage virtual outreach and support for individuals with mental health conditions and substance use disorders, facilitating their navigation to necessary care and services.

Key responsibilities:

• Conduct initial and ongoing virtual outreach to engage individuals and assess their needs and barriers to care
• Utilize motivational interviewing techniques to identify care goals and screen for unmet behavioral health and social service needs
• Support individuals in navigating to appropriate behavioral health and primary care services, ensuring post-care follow-up

Required qualifications:

• 2+ years of experience as a Community Health Worker, Social Worker, or in a patient-facing healthcare role; active certification or licensure preferred
• 1-2 years of experience in patient outreach and engagement, building trust-based relationships
• Experience in a metric-driven environment with a proven track record of meeting performance indicators
• Access to a quiet, HIPAA-compliant workspace with reliable internet connectivity
• Demonstrated experience working with individuals facing complex chronic needs, including mental health and substance use disorders
