Licensed Behavioral Health Social Worker
Job Summary
Focusing on Transitions of Care support, the part-time Licensed Behavioral Health Social Worker will provide short-term interventions for members transitioning to community-based behavioral health services, aiming to prevent unnecessary hospitalizations, while working fully remotely.
Key responsibilities:
- Perform comprehensive psychosocial assessments via phone and video with members and caregivers
- Conduct clinical social work interventions, including brief therapeutic support and safety assessments
- Collaborate with interdisciplinary teams to ensure cohesive, patient-centered care for high-needs patients
Required qualifications:
- Master's degree in Social Work with an active clinical license (LCSW, LMSW, or equivalent)
- Minimum of 5 years of experience in behavioral health or care management
- Competence in performing comprehensive psychosocial assessments remotely
- Able to manage a fully remote work schedule, Monday to Friday, 8:30 am to 5:00 pm PST or MST
