Licensed Care Coach (RN, LPN, LVN)

Job Summary

A company is looking for a Care Coach (RN, LPN, LVN) to engage members in health and wellness coaching.

Key Responsibilities

• Engage and maintain members in active participation in the health program and coordinate member care
• Utilize motivational interviewing and skill-building techniques to support members' health goals
• Collaborate with care team members to improve quality of care and enhance program development

Required Qualifications

• Preferred Associate's or Bachelor's degree in Nursing, or equivalent education/experience
• Extensive experience in a healthcare consumer-facing environment and implementing care plans
• Experience working with the Serious Mental Illness population
• Background or interest in mental health and behavioral health is preferred
• Self-driven with strong organizational and decision-making skills
