Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator to support patient care management.

Key Responsibilities

• Engage high-risk patients to ensure follow-up appointments and close care gaps
• Collaborate with care team members and external vendors to facilitate patient care
• Request medical records to ensure continuity of care for patients

Required Qualifications

• 3+ years of experience in a medical office or healthcare setting
• 2+ years of experience as a Licensed Practical Nurse (LPN), Medical Assistant (MA), or Community Health Worker (CHW) preferred
• Comfort with Microsoft suite and electronic health records
• Ability to engage diverse populations with cultural sensitivity
• Willingness to learn and take initiative in a dynamic environment
