Job Summary
Working remotely in the community areas of Clovis and Portales, NM, the full-time Licensed Care Coordinator - NM will coordinate care for clients with behavioral health conditions through assessment, care planning, and monitoring to ensure quality outcomes and member satisfaction.
Key responsibilities
- Conduct in-depth health risk assessments and develop comprehensive care plans for members requiring intensive interventions
- Implement, coordinate, and monitor strategies to improve health and quality of life outcomes for members and their families
- Collaborate with interdisciplinary care teams to address care issues and ensure effective delivery of services across the continuum
Required qualifications
- 3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in lieu of a degree
- Experience in utilization management, quality assurance, or community health
- Knowledge of referral coordination to community and private/public resources
- Valid driver's license in the state of residence
- Preferred certifications include CCM, LCSW, or RN licensure
