Key Responsibilities

• Engage with members telephonically to coordinate care, dedicating 50-75% of the day to this task
• Develop individualized care plans in collaboration with members, addressing their specific needs
• Maintain accurate documentation of care management activities in the member's electronic health record

Required Qualifications

• 2+ years of experience in a health-related field
• 2+ years of customer service experience
• Proficient in Microsoft Office Suite and capable of effectively utilizing these tools
• Access to a private, dedicated workspace for conducting work effectively
• Associate's degree in a health care-related field or Practical Nurse degree/certificate with active state licensure
