Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation
Develop care/service plans in collaboration with healthcare teams to support member transitions
Facilitate education and resources for members and caregivers to enhance wellness and care quality

Required Qualifications

Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing
2 - 4 years of related experience
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
Registered Nurse strongly preferred
RN Compact License/Multi-State License a plus
