Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation
Develop and collaborate on care/service plans with interdisciplinary teams to support member transitions
Facilitate communication and services between members, providers, and resources to meet healthcare needs

Required Qualifications

Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
2 - 4 years of related experience
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
Weekend availability is required
Flexible scheduling options available
