Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation
Develop care/service plans and collaborate with healthcare teams to support member transitions
Facilitate education and resources to promote wellness and improve quality of care

Required Qualifications

Master's degree in Behavioral Health, Social Work, or an accredited Nursing degree
2-4 years of related experience in care management
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
Ability to work weekends and within the Central Time Zone (CST)
