Key Responsibilities

• Assess member needs across various health and social domains
• Oversee the development of client care plans and facilitate access to services
• Advocate for clients with healthcare professionals and maintain documentation

Required Qualifications

• 2+ years of experience as a care manager, care navigator, or community health worker
• Experience working in a remote setting
• Knowledge of government and community resources related to social determinants of health
• General computer skills and familiarity with Google Workspace and MS Office
• Bilingual (English/Spanish) is a plus
