Licensed Care Manager

Job Summary

A company is looking for a Care Manager, Health Management.

Key Responsibilities

• Provide care management and referrals based on clinical assessments and member needs
• Develop action plans, educate members on managing chronic conditions, and coordinate care
• Monitor ongoing care plans, assess health status, and engage with members and their families

Required Qualifications

• At least 2 years of experience in health care, including 1 year in direct patient care or managed care
• Active and unrestricted clinical licensure (LVN, LPN, LCSW, LPC, or RD) as required by state regulations
• Knowledge of community resources and ability to work with diverse populations
• Ability to work independently with minimal supervision and demonstrate self-motivation
• Excellent time-management and prioritization skills
