Licensed Care Manager

Job Summary

A company is looking for a Care Manager.

Key Responsibilities

• Develop trusting relationships with members to support health behavior changes using motivational interviewing techniques
• Assist in coordinating care and provide educational resources and recommendations for follow-up care
• Create individualized health plans and serve as a clinical resource for Care Management team members

Required Qualifications, Training, and Education

• Bachelor's Degree in Occupational Therapy (OTR) or Associate's Degree (OTA or PTA)
• Minimum 2-4 years of clinical experience with patient assessments and care plan creation
• Experience supervising OTAs and working with the Medicare population
• Familiarity with Motivational Interviewing techniques and behavioral change theory
• Experience working with geriatric or frail populations
