Licensed Care Manager

Job Summary

Coordinating and overseeing behavioral health services, the full-time Licensed Care Manager will ensure members receive high-quality care while promoting engagement and recovery in a remote environment.

Key responsibilities

• Conduct holistic assessments and develop individualized care plans to support recovery and self-management
• Build collaborative relationships with members and providers to enhance engagement and continuity of care
• Ensure compliance with clinical, regulatory, and organizational standards through accurate documentation and participation in quality improvement initiatives

Required qualifications

• Current, unrestricted state license as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse
• 3-5 years of direct clinical care experience in behavioral health or substance use treatment settings
• Strong clinical judgment and case management skills for complex care transitions
• Proficiency in Microsoft Outlook and Word for real-time documentation
• Ability to pass a comprehensive background check upon hire and throughout employment
