Job Summary

A company is looking for a Lead Care Navigator, Remote US.

Key Responsibilities

Deliver hands-on care navigation services and conduct SDOH screenings for patients
Partner with leadership to design workflows, SOPs, and standards of care for the navigation program
Identify gaps in care and contribute to the growth and mentorship of future team members

Required Qualifications

Clinically grounded LVN preferred; RN considered with leadership potential
2-5 years of experience in care navigation, case management, or a related patient-facing role
Fluency in social determinants of health and experience connecting patients to resources
Comfortable with care management platforms and digital tools
Experience in a startup or pilot program is a plus
