Licensed Care Navigator - Mood & Anxiety

Job Summary

A company is looking for a Care Navigator - Mood & Anxiety.

Key Responsibilities

• Conduct telephonic and telehealth screenings, brief interventions, and referrals for members with mood and anxiety disorders
• Utilize motivational interviewing and DBT skills to support members in their care journey
• Coordinate care with providers and community resources, ensuring appropriate levels of care are identified

Required Qualifications

• Independently licensed mental health professional (LCSW, LMFT, LPC)
• Minimum of 2+ years of clinical care experience with mood and anxiety disorders in adult and adolescent populations
• Experience with motivational interviewing and diagnostic assessment
• Knowledge of best practices for mood and anxiety disorders, including CBT and DBT
• Experience working with diverse populations and collaborating with families
