Job Summary

Managing an active caseload of patients, the full-time Licensed Care Navigator - PA, NJ, NY, DE will develop individualized care plans, provide emotional support, and navigate public benefit systems while working remotely.

Key responsibilities

• Develop and maintain individualized care plans for patients, providing consistent support and addressing practical and emotional barriers
• Research and coordinate resources based on patients' specific needs, ensuring timely follow-up and resolution
• Document all interactions and collaborate with the clinical team to ensure comprehensive patient care

Required qualifications

• 2+ years in care navigation, case management, or community health with experience in virtual or phone-based settings
• Direct experience navigating social determinants of health, including housing, transportation, and behavioral health resources
• Knowledge of community resources and systems in Pennsylvania, New Jersey, New York, and Delaware
• Proficient in using EMR or care management platforms with strong overall tech savviness
• High school diploma or GED required; associate's or bachelor's degree in social work, public health, or human services preferred
