Job Summary

Managing and coordinating home care services remotely, the full-time Licensed Care Specialist will ensure compliance with LHCSA regulations, facilitate communication among patients and caregivers, and oversee service delivery to maintain high-quality patient care.

Key responsibilities
  • Coordinate and manage patient care in accordance with LHCSA regulations and individualized care plans
  • Serve as the primary point of contact for patients, providers, and home health aides, ensuring clear communication
  • Monitor compliance with Electronic Visit Verification (EVV) requirements and address any service gaps or discrepancies
Required qualifications
  • High school diploma or equivalent required (Associate's or Bachelor's degree preferred)
  • 1+ years of customer service experience in a high-volume call center, healthcare, or homecare agency
  • Experience with CRMs or Workforce Technology such as Salesforce or Medflyt
  • Strong organizational skills with the ability to manage time effectively
  • Computer proficiency with strong typing skills and the ability to multitask while resolving customer issues
