Job Summary

A company is looking for a Case Manager.

Key Responsibilities:

• Create and promote adherence to treatment plans developed by healthcare providers
• Monitor and document patient progress, ensuring continuity of care and effective treatment
• Connect patients to community resources to enhance health and well-being

Qualifications:

• Current standing as a licensed practical nurse (LPN) is required
• 5 years of experience in a clinical or community resource setting/care coordination
• Case Management experience is preferred
• Highly organized with the ability to maintain accurate records
• Proficient computing skills and ability to learn new systems
