Licensed Case Management Nurse

Job Summary

Seeking a compassionate and detail-oriented Licensed Case Management Nurse, this remote position will manage patient care by promoting adherence to treatment plans, maintaining clinical reports, and connecting patients with community resources to enhance their health and well-being.

Key responsibilities:

• Create and promote adherence to treatment plans developed by healthcare providers
• Monitor patient progress and document adherence to treatment plans in detailed clinical reports
• Serve as a liaison and advocate for patients, care teams, and community resources to ensure continuity of care

Required qualifications:

• Current standing as a registered nurse (RN) or licensed practical nurse (LPN)
• 5 years of experience in a clinical or community resource setting/care coordination
• 2 years of case management experience
• Highly organized with strong record-keeping abilities
• Proficient computing skills and exemplary telecommunication skills
