Job Summary

Key Responsibilities

• Telephonically manage comprehensive cases and coordinate care per established guidelines

• Advocate for patients by facilitating quality care delivery and implementing cost management strategies

Required Qualifications, Training, and Education

• Graduation from an accredited college with a degree in Social Work required
• License in Social Work preferred
• Minimum 2 years of acute healthcare experience, including 2 years of case management experience
• Basic computer literacy and proficiency in Microsoft Office applications
• Knowledge of medical claims and coding systems such as ICD-10, CPT, and HCPCS
