Job Summary

To support home health referrals, the full-time Licensed Clinical Account Executive will coordinate patient care, manage communication between healthcare providers, and execute sales strategies to enhance market share while working remotely.

Key responsibilities

• Coordinates patient referrals to home health services and evaluates suitability upon physician request
• Acts as a liaison between patients, caregivers, physicians, and the home health agency, ensuring effective communication
• Provides educational in-services about home health services to healthcare professionals within facilities and physician offices

Required qualifications

• Formalized clinical training from an accredited institution
• Licensed in the state of practice
• Experience in home health care
• Proficient in Microsoft Outlook and CRM software
• Current Driver's License and access to a dependable vehicle or public transportation
