Job Summary
Leading clinical strategy and oversight, the full-time remote Senior Director of Clinical Care Coordination will manage care coordination, clinical escalations, case management, and client placement across the CCS network, ensuring high-quality, person-centered care delivery.
Key responsibilities
- Lead and manage specialized care and clinical case management teams to align with strategic objectives
- Oversee client placement processes and referral pathways to ensure timely and equitable access to services
- Analyze clinical trends and performance data to optimize clinical operations and service delivery
Required qualifications
- 8+ years of experience in a home-health setting with supervisory responsibilities
- Licensed Clinical Psychologist, LMFT, LPCC, or LCSW required
- 5+ years of progressive leadership experience in clinical operations
- Experience working with individuals with autism spectrum disorders (ASD) and related developmental disabilities
- Proven success in strategic planning and program development
