Licensed Clinical Care Navigator

Job Summary

A company is looking for a Senior Clinical Care Navigator.

Key Responsibilities:

• Own the end-to-end success of the service line, ensuring coordinated and timely patient care
• Manage complex care coordination scenarios for seamless patient journeys
• Coach and mentor junior team members while identifying operational gaps for improvement

Qualifications and Requirements:

• 7+ years of clinical experience in Substance Use Disorder and Behavioral Health with relevant licensure
• At least 3+ years of direct patient care or coordination experience
• Substance Use Disorder certification is strongly encouraged
• Spanish speaking is preferred
• Experience in broader behavioral health, including eating disorders and acute mental health needs, is a plus
